LIABILITY WAIVER, CONTRACT & PAYMENT INFORMATION
PLAYER LIABILITY WAIVER
I hereby authorize the staff of Strictly Hoops to act for me according to their best judgment in any emergency requiring medical attention. I, the parent/legal guardian of the above named student-athlete, hereby acknowledge that participation in Strictly Hoops, as in any sport, may result in accident and/or injuries. Although there are risks involved, I still give my approval for the above named student-athlete to participate in any and all Strictly Hoops activities. I expressly assume all risks and hazards incidental to such participation, and I do hereby waive, release, absolve, indemnify and agree to hold harmless Strictly Hoops, its staff, suppliers, sponsors, participants for any claim arising out of injury or illness to the said camper, regardless of the cause.
PARENT/STUDENT-ATHELETE CONTRACT
All parents and players are required to review the Parent/Student-Athlete contract prior to registering, understanding that they will be responsible for enclosed content.
AGREEMENT
By digitally signing you have agreed to the Player Liability Waiver, Parent/Student-Athelete cotatract and understand there is a non-refundable registration fee of $75 required for registration to be complete.
